Job Description
This is a different way of helping people. This is an opportunity for you to use your nursing skills and experience to educate caregivers on the physical health needs of their patients who live in the community and have a mental health diagnosis. You will be teaching their caregivers how to give them a better quality of life. You also will have autonomy over your nursing practice with less physical demand on your body than working in a hospital or skilled nursing facility. Give back by teaching others.
You will spend your day in the community at AFCs, private homes, ABA centers, day programs, camps, and sometimes at the office in our outpatient clinic giving injections and helping out with the psychiatric clinic.
Under general supervision of Supervisor of Business Practices-Health Home and clinical supervision of the Chief of Health Services and Integrated Care, provides nursing services to consumers of Saginaw County Community Mental Health Authority (SCCMHA) in a variety of office and community-based settings, as assigned. This position serves adults and children with multiple physical health conditions, mental illness, substance use disorder(s) and/or developmental disabilities. Nursing services are provided according to the American Nurses Association and Michigan Nurses Association scope and standard of practice and the Michigan Public Health Code and Michigan Mental Health Code. This position will be knowledgeable about and actively support culturally competent recovery-based practices; person centered planning as a shared decision-making process with the individual, who defines his/her life goals and is assisted in developing a unique path toward those goals; and a trauma informed culture of safety to aid consumer in the recovery process.
