How You'll Make an Impact
As a key player in our interdisciplinary care team, you will:
- Assess the Whole Person - Conduct comprehensive psychosocial evaluations to understand behavioral health needs, mental status, and social determinants affecting care.
- Deliver Targeted Behavioral Health Support - Provide short-term, evidence-based interventions (Motivational Interviewing, Critical Time Intervention, Trauma Informed Care) to help patients manage mental health challenges.
- Be a Crisis Stabilizer - Offer timely crisis intervention, coordinating higher levels of care when needed.
- Connect Patients to the Right Resources - Help patients navigate community services for social support, housing, substance use treatment, and more.
- Educate and Empower - Serve as a trusted resource for patients, families, and healthcare providers, offering guidance on behavioral health conditions and treatment pathways.
- Drive Care Coordination - Work alongside primary care providers, health advocates, and community resources to ensure patients receive comprehensive, well-integrated care.
- Document and Improve - Maintain clear, compliant clinical records and contribute to ongoing quality improvement efforts.
Who You Are
You're a compassionate, resourceful, and clinically skilled mental health counselor who thrives in dynamic, community-based care settings.
We'd love to hear from you if you have:
- LMHC with 1-3 years of post-master's clinical experience in behavioral health, integrated primary care, or community-based settings.
- Experience working with serious mental illness (SMI), substance use disorders (SUDs), and co-occurring conditions.
- Strong skills in psychosocial assessments, crisis intervention, and treatment planning.
- A collaborative mindset-comfortable working alongside medical providers, care coordinators, and community partners.
- A solution-focused, proactive approach to helping patients navigate care barriers.
- A high level of clinical judgment, adaptability, and organizational skills.
Where You'll Work: A Hybrid Model That Puts Patients First

- 60% Field-Based Care - Meet patients where they are, whether it's their home, a community organization, or via telehealth.
- 40% Remote Work - Use this time for documentation, care coordination, and virtual collaboration with the team.
- Reliable personal transportation is required (and the ability to travel using our mobile health unit). Mileage reimbursement is provided.

Compensation & Benefits That Support You
Compensation: $70,000-$95,000 based on experience
