Substance Abuse Counselor

Changing Phases Behavioral Health is seeking to immediately hire Substance Abuse Counselors that are CAC certified.

POSITION REQUIREMENT: 

Candidate must have an undergraduate degree in a related field and be CAC certified or hold an LPC, LCSW license.  If Counselor is not a CAC, LPC or LCSW, that Counselor will be required to diligently work toward certification while in the employment of Changing Phases Behavioral Support. 

Candidate must be willing to work Monday-Friday from 5:00 am-1:00 pm. 

Intake

• Evaluate patient and determine appropriateness of MOUD admission per State and Federal criteria.
• Provide information about MOUD and alternative treatments to ensure that the patient is able to give informed consent.
• Review program rules and regulations; provide basic program orientation
• Complete psychosocial and treatment plan appropriate to patient’s condition

Treatment

• Work with patient to develop individualized treatment plan and goals
• Obtain urine drug screens for every patient at least once a month on a random schedule.  
• Annually obtain proof of residence and proof of income from every patient on caseload.
• Complete in the appropriate time frame treatment plan reassessment for every patient.
• Complete and submit all patients’ requests to the MD.
• Make certain that patient records are always in the proper order and are stored in a confidential manner
• Meet with assigned patients as determined
• Record all counseling sessions appropriately for documentation purposes.

Additional Responsibilities

• Arrange guest dosing for patients as needed 
• Complete minimum required trainings as directed.
• Conduct the Patient Orientation class as required.
• Participate in weekly treatment team meetings with clinical staff.
